Request Forms

Use these forms to request information resources that are not available in Allara Library’s print, microform or electronic collections. These services are available to all currently enrolled University of Pikeville students and currently employed University of Pikeville faculty and staff.

Students may request up to five items per week and faculty/staff may request up to 10 items per week. If your requests exceed these limits, your remaining requests will be processed the following week.

Any request for items available electronically or in the library will not be processed.

Please note that all patrons are expected to return their interlibrary loan (ILL) items on or before the date due. If you fail to return your items on time, your ILL privileges will be suspended for a period of one calendar year. It is often possible to renew interlibrary loans, if the request is made in advance. Please make renewal requests at least one week before the due date.
